Update Custom Targeting Modal to have buttons always visible

I cannot count the number of times I've forgotten to hit save for a new Custom Targeting attribute because the Save button isn't immediately present. I'd suggest that the attributes get moved into a div with overflow to allow scrolling and fix the buttons at the bottom so they're always visible.
